Service fees
Services will be discussed and applicable service fees mileage/weight/footage etc, and other service fees will be detailed at this time and documented in a memorandum of understanding and service level agreement. Retainer
A retainer of $330 is due for strategic healthcare logistics services at the signing of service level agreement and memorandum of understanding, $165 of this is not recoupable. Unused quantities of the retainer will be returned to customers after services are complete, in the event customer exceeds this quantity, $165 retainer will be assessed, and returned if not completely used again, customer billed at 17.25 per 1/4 hour for recoupable quantity of retainer. Customer is also billed according to applicable miles and weights, these are calculated with GPS and scale. Only patient transit terms and STAT vary from other services. STAT uses retainer; urgency fee is billed at $17.25 per 1/4 hour, and customer is also billed according to applicable miles and weights, these are calculated with GPS and scale, customer receives unused quantities of retainer back at completion of delivery, in the event customer exceeds retainer, customer is billed at $17.25 per 1/4 hour. High - Volume Partnership Rates
A retainer of $5000 a month is due for all selected service, $500 of this is non-recoupable each retainer of $5000. Partners with leftover retainers from previous month will have a positive balance forward and will not incur this retainer again unitl the prior retainer runs out. Customers are sent a bill at the end of the month, and charges are subtracted from retainer. If customer chooses to continue services, the balance will be carried over to the following month, if they choose to cancel, the unused retainer will be returned to them within 1 business day, with no forfeitures for liquidated damages, preparation and lost opportunity, Retainers can be modified to weekly or bi-weekly services. Modifications
Customers may modify their service daily in writing by 5PM the day prior. These modifications do not change any cost or associated fees. Please note that customers cannot add tasks to their commitment unless a $330 retainer or $600 deposit is paid. Please visit www.wellcloudgroup.org
